New Patient Registration
You have been scheduled to see either Dr. Marc Urquhart or Dr. Erica Urquhart. To help make your visit as efficient and comfortable as possible, please provide your email address to our staff when scheduling your initial appointment. Please bring with you any prior medical records (reports, history, x-rays). It is now a REQUIREMENT for all new patients to complete registration online through the Patient Portal. Comfortable clothing (such as shorts and a sports bra) is recommended during examinations.
